About this grant

The purpose of this program   is to: promote full integration and collaboration in clinical practices between physical and behavioral health care; support the improvement of integrated care models for physical and behavioral health care to improve overall wellness and physical health status; and promote the implementation and improvement of bidirectional integrated care services, including evidence-based or evidence-informed screening, Grant Number: SM-26-021. Eligibility requirements

All of the following criteria must be met:

• Eligibility for this program is statutorily limited to a State or appropriate State agency. Appropriate state agencies include the state mental health authority, the single state agency (SSA) for substance abuse services, the State Medicaid agency, or the state health department.
